Prostatomegaly Meaning

Prostatomegaly is a condition in which prostate gland become bigger in size. Usually prostate gland in men start increasing in size after 40 years of age. Benign prostate hyperplasia (enlarged prostate gland) is another name of prostatomegaly. Prostate gland is an important and main part of male reproductive system. Prostate gland is muscular and is small in size but covers urethra. Prostate gland is responsible for production of semen in men. It surrounds urethra in men. As a result no urine passes from urethra and leads to urinary retention. When a low amount of urine is passed and larger amount of urine stays inside bladder it leads to chronic urinary retention. Prostatomegaly Definition


Prostatomegaly is a condition defined as enlargement of prostate gland. It is also known as benign prostatic hyperplasia BPH. It may occur due to enlargement or multiplication of cells in prostate gland. The gland is an important part of male reproductive system. Without this men cannot reproduce because it is responsible for producing semen in men. Semen is important for the reproduction in men. Diagnosis of prostatomegaly can be done with physical examination. A digital rectal exam can be conducted for the purpose of diagnosis. In physical examination, the physician inserts finger inside rectum and feels the size of rectum. By touching, physician can roughly estimate the size of prostate gland. If there are lumps and hard mass in the prostate gland then these indicate the presence of cancer in prostate gland.

Analysis of urine is another factor which indicates the presence of prostatomegaly in men.

Prostatomegaly Grade 1

Prostatomegaly is a medical condition in which the size of prostate gland increases. Prostatomegaly usually occurs after 40 to 50 years of age in men. Benign prostate hyperplasia BPH is another name of prostatomegaly. It is the enlargement of prostate gland. Prostate gland is an important part of men reproductive system. Without this gland, men are not fertile and can't reproduce. It is present around urethra and is small in size.


It has a muscular texture and is soft on touch. Prostate gland is responsible for producing semen in men. Prostatomegaly occurs when cells of prostate gland start multiplying and as a result it's size increases up to a dangerous level that it compresses urethra. Due to compression of urethra, the flow of urine becomes limited. Prostatomegaly is not a cancerous condition and it has nothing to do with cancer. Even it does not lead to cancer.

Talking about the grading of prostatomegaly, it is dependant on the size of prostate gland. The size of prostate gland start increasing slowly when men reach at the age of 40 or 45. Usually it does not harm urethral flow of urine but after 40 or 45 years of age, the cells of prostate gland start multiplying and make it swell. There are markings of size of prostate gland which can seen in the radiographic imaging of prostate gland. The size of 33cc is the marking that prostate gland has increased in size than usual. 33 cc means 30 grams. This size classified prostatomegaly into different grades i.e. grade 1, grade 2 and grade 3. These grades are specific about their symptoms and their treatment plans.

The intensity of symptoms start increasing from grade 1 to grade 3.
In grade 1 of prostatomegaly, the symptoms are mild. These symptoms  include decrease flow of urine due to increased size of prostate gland. Other than decreased flow of urine, the urination time may increase. Patient used to wait longer for urine to flow. The symptoms are mild enough that these do not disturb the routine life of patient. No specific treatment is required for that purpose but some lifestyle changes helps a lot to cover these symptoms.

As grading system of prostatomegaly is dependent on the size of prostate gland. so the size of prostate gland in grade 1 prostatomegaly is increase in 25 percent of original size of prostate gland.
Some patients who are not satisfied with their life style modifications, usually use medications for grade 1 prostatomegaly. These medications provide benefits but also pose some side effects. As compared to other two grades of prostatomegaly, grade 1 is easy to treat. People with grade 1 prostatomegaly does not have to worry about it because it is harmless. But if patient does not follow some life style modifications or some medications then it may transform to grade 2 and then grade 3 and the symptoms keep going severe. And the treatment will not be an easy task.


Prostatomegaly Grade 2

Prostatomegaly is a medical condition in which the prostate gland increases in size. It is also named as benign prostate hyperplasia (enlargement of prostate gland). It usually occurs after 40 years of age in men. After 40 to 45 years of age, the cells of prostate gland start multiplying and the size of prostate gland start increasing.


Due to increase in size the prostate gland covers the urethra. And as a result the urine flow is disturbed and causes urinary retention. When urethra is compressed, the low volume of urine is passed and much volume of urine remains in bladder and this is called urinary retention.  Along with this the urination time also increases. Prostate gland is a muscular structure and is small in size. It is soft on touch and its physical examination is easy.

According to size of prostate gland in prostatomegaly, it is classified in to different grades i.e. grade 1, grade 2 and grade 3. The symptoms and treatment options are different in all grades. The severity of symptoms increases moving from grade 1 to grade 3. In grade 2 prostatomegaly, the symptoms are at moderate level.

The size in grade 2 prostatomegaly is 50 percent more than usual. The frequency of urination increases and the volume of urine decreases. After urination, the patient does not become satisfied because he feels his bladder full even after urination. In case of grade 2 prostatomegaly, the medications are used instead of life style modifications. There are some alpha blockers which are used for its treatment.

These alpha blockers include prazosin, doxazosin, alfuzosin, terazosin and tamsulosin. Hormonal therapy is also used to treat grade 2 prostatomegaly. So the disturbed hormones can be treated with dutasteride and finasteride. Hormonal therapy is effective and help a lot in reversing the size of prostate gland. The alpha blockers and hormonal therapy collectively works against prostatomegaly.

These medications will control the symptoms and the size of prostate gland so it is considered the most effective treatment in combination. The diagnosis have a main role in judging the size of prostate gland and selecting the right treatment for it.

Prostatomegaly Grade 3

Prostatomegaly is a medical condition in which the size of prostate gland increases. Prostatomegaly is also known as benign prostate hyperplasia BPH. Prostatomegaly is the enlargement of prostate gland. Prostate gland is the main organ of men reproductive system. Without prostate gland, men are not fertile because it is responsible for production of semen. And semen is essential for reproduction. Prostatomegaly occurs after 40 to 45 years of age.


After 40 years, the cells of prostate gland start multiplying and it looks swelled. It is present right next to urethra. When prostate gland increases in size, it covers the urethra and as a result urinary retention occurs. Urinary retention is a condition in which low volume of urine passes from urethra and maximum volume remains in bladder. On the basis of increase in size of prostate gland, prostatomegaly is divided in to three main grades i.e. grade 1, grade 2 and grade 3.

Prostatomegaly is confirmed when the size of prostate gland increases up to 33 cc. 33 cc is equals to the weight of 30 grams. Diagnosis is easy via two methods i.e. radiographic imaging and physical examination. It is a muscular structure and small in size. It is really soft on touch. All the classified grades are different on the basis of severity of symptoms and the treatment assigned to them. In grade 1 and grade 2 the symptoms are mild and moderate respectively. And their treatment vary according to the symptoms.

But in grade 3 the symptoms are severe and are unbearable and its treatment is also different. The symptoms of grade 3 prostatomegaly causes so many other problems in men. In this grade of prostatomegaly, the medications are of no use and physician recommend surgery. In grade 3 prostatomegaly, the pain become unbearable because the flow of urine is totally blocked from urethra and leads to no urination. The size of prostate gland in grade 3 has increased more than 50 percent of the original size.

Patient does not feel comfortable and experience unbearable pain in lower abdominal region. He is not able to walk around freely or lay down on bed because of pain. So on emergency note, surgery is recommended. Now talking about surgical removal of prostate gland, there are some surgical procedures by which removal can be done easily. There are total three main surgical procedures named water induced thermotherapy, transurethral nasal ablation and transurethral microwave therapy. Water induced thermotherapy can be performed easily in the room of physician to the outpatients.

Patient does not have to be admitted in hospital. Another main procedure by which surgery can be performed is transurethral resection of prostate. The selection of surgical procedure is dependent on the diagnosis. Physical examination helps a lot in diagnosis. In case of in patients, two types of surgical procedures are performed i.e. prostectomy and transurethral incision of prostate gland. These two procedures require time for the proper recovery that's why patient have to be admitted in hospital.

Prostatomegaly Causes

Prostatomegaly Mild

Prostatomegaly is the enlargement of prostate gland which is present near urethra. Prostate gland is small in size and muscular in nature. It can be easily examined in physical examination. Prostate gland when increase in size it compresses urethra. Due to compression, the urethra is not able to pass greater amount of urine and this leads to urianry retention. Urinary retention is a condition in which the low volume of urine is passed through urethra and most of the volume remains in bladder. This is the main complication regarding prostatomegaly. Talking about symptoms of prostatomegaly, it is divided in to three main types i.e. mild, moderate and severe prostatomegaly. And on the basis of these three categories, these are termed as grade 1, grade 2 and grade 3 prostatomegaly respectively.

In mild state or grade 1 prostatomegaly, the symptoms are mild and does not require treatment. There are some lifestyle modifications which should be followed to cover mild symptoms of prostatomegaly. Prostatomegaly is the increase in size of prostate gland, so when the size enlarges from 33 cc, it is counted as prostatomegaly. 33 cc is equals to 30 grams. When prostate gland start multiplying, the mild symptoms start appearing. Symptoms like weak stream of urine, leakage of urine or inconvenience, nocturia (i.e. the need of urination increases almost 2 to 3 times per night), patient may start experiencing dripping of urine at the end of urination everytime.

The bladder is not completely empty, because of compression of urethra. Sometimes patient find it hard to control urine because he feels sudden urge to urinate. The stream of urine is weak. Patient experience pain in lower abdominal region and in urethra during urination. This pain is mild and bearable. Patient does not have to take medication. This is the mild state of prostatomegaly which is also known as grade 1 prostatomegaly. These symptoms does not require treatment and can be treated by following some life style modifications.

These modifications include quiting of  medications which are responsible for causing prostatomegaly. These medications has a side effect i.e. enlargement of prostate gland. The enlargement of prostate gland also leads to fertility issues. Prostate gland is responsible for the production of semen in men. Semen is important for the transfer of sperms to the ovary of women. And forms zygote.

In prostatomegaly, the prostate gland increases in size and the production of semen is disturbed. Ultimately it leads to the infertility in men. Infertility is the condition achieved in severe form of prostatomegaly. Due to decreased production of semen, there may be hyperspermia or hypospermia. In hyperspermia, the concentration of sperms is higher but dead and in case of hypospermia, there is low volume of sperms leading to infertility and many other issues.

  • Patient experience dripping at the end of urination every time. 
  • Patient have to wake up at night for more than 5 times to urinate. 
  • Patient may experience bleeding during urination. 
  • The color of urine changes. 
  • The stream of urine gets weaker than before. 
  • The smell of urine becomes toxic and unbearable. 
  • Patient may experience pain in lower abdominal region due to prostatomegaly. This pain is sometimes unbearable and causes problems in regular routine of patient. 
  • The bladder is not fully empty and patient does not feel comfortable. 
  • Patient experience the need of straining during urination. 
  • The patient suddenly feels the necessity to urinate. And this sudden urge sometimes make patient embarrass. 

There May be blood in urine. Which is the exact indication of some problems in bladder and urethra.
Problem in the fertility of men, is the rightly indication of problem in prostate gland. As it is responsible for production of the semen in men. So in prostatomegaly, the production is disturbed and as a result it leads to various problems like hypospermia ( low volume of sperms or semen per discharge).

Prostatomegaly can make patient infertile. And men will not be able to reproduce babies.
The treatment for grade 2 prostatomegaly or moderate prostatomegaly is different from grade one prostatomegaly or mild prostatomegaly because, in later one , only life style modifications are preferred. In grade 2, the medications are also use along with lifestyle modifications.

So hormonal therapy is also used to treat moderate prostatomegaly. Hormonal therapy include medications like finasteride and dutasteride which reduces the level of hormones.

Prostatomegaly Severe

Prostatomegaly is a condition in which prostate gland increases in size up to an abnormal level. Prostatomegaly occurs at the age of 40 to 45 years. Prostatomegaly is the rare condition and does not cause harm at initial stage. Prostate gland is present right next to urethra and when the size of prostate gland increases, it causes problem in the flow of urine. Prostatomegaly is confirmed when the size of prostate gland increases more than 33 cc. 33 cc is equals to 30 grams.


But it is the mild form of prostatomegaly. On the basis of symptoms and the size of prostate gland, prostatomegaly is divided in to three main categories i.e. mild , moderate and severe. In mild form of prostatomegaly, the symptoms are mild and do not cause problems in routine life of patient. This form of prostatomegaly can be treated easily by following some life style modifications. These life style modifications leads to an easy way of controlling mild symptoms of prostatomegaly.

Then talking about moderate prostatomegaly, it includes symptoms at moderate level i.e. pain which causes discomfort, problems related to urinary flow and bladder. It also causes problems in the fertility of men. These symptoms do not require only life style modifications but also require medications. These medications are of great effect on prostatomegaly. These medications include alpha blockers. Other than these, hormonal therapy is also used to treat prostatomegaly. In severe prostatomegaly, the symptoms gett worse.

These symptoms include complete blockage of the urine and leads to worse urinary retention. The symptoms are not under control. Patient experience unbearable pain in the lower abdominal region. This pain makes him really uncomfortable and irritating. The treatment include the surgical removal of prostate gland from body. It is recommended so because, the medications are of no use anymore. Medications cannot control these symptoms because these are out of control in severe prostatomegaly. It is also termed as grade 3 prostatomegaly.

There are different tyeos of surgeries which are performed to remove prostate gland. Some of them are so easy that it can easily be performed in out patients and some of them require time for healing. Surgical procedures which are usually performed in case of out patients include transurethral microwave therapy, water induced thermotherapy and transurethral nasal ablation. Other than these, there are some surgical technique used in case of in patients and these techniques include simple prostectomy, transurethral resection of prostate and transurethral incision of prostate. All the techniques used for in patients, require some time for healing.

That's why patient have to take admission in hospital. The recovery time depends upon the technique and severity of condition. Surgery is considered as the last option for treating prostatomegaly. It is preferred when medications are of no more help. There are some complications which are attached to the surgical removal of prostate gland. And the most common of them is the loss of fertility in men. Because prostate gland is responsible for the production of semen in men.

Prostatomegaly Symptoms

Prostatomegaly is the enlargement of prostate gland up to an abnormal level. The symptoms start appearing when the size of prostate gland increases in size more than 33 cc. 33 cc is equals to 30 grams. The symptoms at starting are mild and can be easily cured. But after some time, the symptoms become a bit worse and this phase is known as moderate prostatomegaly. But when the symptoms become out of control and become unbearable then it is termed as severe prostatomegaly. On the basis of symptoms, it is divided in to three main categories i.e. mild, severe and moderate. The treatment are also designed according to the category diagnosed. Talking about the symptoms of prostatomegaly, some specific ones are discussed below:


Patient experience pain in the lower abdominal region. Pain may be mild, moderate or severe according to the size of prostate gland. Mild pain is bearable while moderate need medications and severe one require sudden treatment. Patient does not feel satisfied even after urination. Because the bladder is not fully empty and incomplete bladder emptying leads to irritation.

Patient may suffer from nocturia. Nocturia is a condition in which patient feels to urinate more than 3 to 4 times at night. This makes patient really uncomfortable. Everytime when patient urinate, the stream of urine is weaker than usual and patient notice dribbling at the end of urination. This is the clear symptom of prostatomegaly.

Incontinence occurs. It means patient cannot control his urine for longer time and it leaks out. It makes patient embarrass in front of public. Patient have to strain it while urination. Because it hurts when urine passes through urethra. The stream of urine is weaker than usual. Pain with weak stream of urine indicates prostatomegaly.

Patient feels sudden urge to urinate. And have no control over urine. The stream of urine is slow and weak. Sometimes patient have to wait for urine to flow. The flow of urine is disturbed. Sometimes patient does not experience continuous stream of urine. The worse form of prostatomegaly can be judged easily when there is blood in the urine. Bloody urine is the indication of severe prostatomegaly.

All these symptoms collectively occurs in patient. The severity of symptoms depend upon the size of prostate gland. The treatment is totally dependent on the type of symptoms. Other than these symptoms there are two main complications which are related to prostatomegaly. 1st one is the urinary retention, in which the bladder is not fully empty. And very little amount of urine is passed from urethra. And most of the volume remains in the bladder.

2nd complication is the infertility in men. As prostate gland is responsible for producing semen in men. When it increases in size, the production of semen is disturbed and as a result the decreased semen leads to decrease sperm count and it becomes difficult to make a women pregnant. This condition is termed as hypospermia.

Prostatomegaly Treatment

Prostatomegaly is the term used to define benign prostate hyperplasia BPH. It is the enlargement of prostate gland up to an abnormal level. It is a natural aging phenomenon and is not considered a serious issue. But if it becomes worse, it starts leading to different complications and problems in body. The two main complications regarding prostatomegaly are urinary retention and infertility issues. The causes behind prostatomegaly are limited but the symptoms are wide. And treatment options also have wide range according to the severity of prostatomegaly. The type of prostatomegaly according to symptoms are mild, moderate and severe prostatomegaly. And treatment is selected according to these types.


There are two types of treatment available 1st one is medical type and 2nd one is surgical treatment.
Other than these, there are some home remedies too which helps a lot in treatment of prostatomegaly. Like patient should not wait long to urinate if he feels sudden urge for urination.
Patient should urinate as soon as possible he feels urge for urination and try to empty the bladder completely.

These home remedies work a lot in treating mild form of prostatomegaly. Because there is no need of medications in mild prostatomegaly. Talking about medical treatment, it is used mainly in case of moderate prostatomegaly. Usually the medications which are prescribed for moderate prostatomegaly specially for the main complication i.e. urinary retention.

Medications include alpha blockers. Common alpha blockers used are prazosin, terazosin and doxazosin. These medications helps to relax the muscles of prostate gland and as a result, the symptoms are treated.

There are some new alpha blockers like silodosin, tamsulosin and alfuzosin. These are commonly prescribed because these have less side effects as compared to previous alpha blockers.
Some medications are used to shrink the size of prostate gland. These medications include dutasteride and finasteride. Due to shrinkage of size of prostate gland, the urinary problems may resolve. These drugs help a lot in case of moderate type of prostatomegaly. These are considered as medications for hormonal irregularities.

  • Another medicine named tadalafil is also used and is recently approved for benign prostate hyperplasia BPH. 
  • Some medications are used to calm down the muscles of bladder. These medications include, oxybutynin, merabegron, oxytrol, darifenacin, gelnique, solifenacin and trospium. 
  • Now talking about the surgical treatment of prostatomegaly, it is used for the severe type of prostatomegaly. Common surgical procedure used is transurethral resection of prostate. It is considered the gold standard. In this surgery, a small incision is made on prostate gland and removed the innermost core of prostate gland. 
  • Another surgical procedure i.e. transurethral incision of prostate , a small instrument is passed through urethra and one or two small cuts helps to reduce the size of urethra. 
  • There is a simple prostectomy, which can be performed in the office of physician. Local anesthesia is used for that purpose. Rezem therapy is used in which a water tream is injected inside prostate gland and it is also known as urolit procedure.
